Health insurance - how and where?

• Hi, i am an australian living in hamburg, and working as a freiberuflich english teacher.
  I desparately need health insurance, but can't seem to get coverage without a niederlassungserlaubnis (i have an aufenthaltstitel already), but no one can tell me what a niederlassungserlaubnis is, or where i can get it. Bezirksamt have told me it is a meldebestatigung, which i have, but that isn't enough for the companies, and the ausslander amt told me it was a aufenthaltstitel, which is also not enough for the companies....
  i am confused and worried. can you give me any advice?
